New Patient Forms & Instructions

Before Your First Visit

To help us prepare for your appointment, please complete the appropriate New Patient Forms below. Be sure to sign where indicated and return them before your scheduled visit—either by faxing them to 478-788-5070 or dropping them off at our office. This ensures we have time to enter your information into our system in advance.

On the day of your appointment, please bring:

• Your insurance card

• A current list of your medications

• Plan to arrive 20 minutes early for check-in

Cancellations & Rescheduling
